1. Mask Policy
   I understand that masks are optional but strongly encouraged for the safety and comfort of all visitors and staff.

2. Access Policy
   I acknowledge that only decision-makers involved in the service being provided are allowed inside the building. This policy is in place to ensure the health and safety of clients and staff amid the ongoing concerns surrounding COVID-19, influenza, and RSV.

3. Responsibility Acknowledgment
   I understand and accept full responsibility for the accuracy of all information I provide and sign off on. Additionally, I agree to pay all applicable fees associated with the services rendered.

4. Surveillance Notice
   I am aware that this office is monitored by 24-hour video and audio surveillance for security and safety purposes.

5. Compliance with Policies
